You're right they did run far more TPH 50 years ago (I remember riding them), but their safety rules weren't as stringent as they are now. They let motormen routinely key by red signals without permission, and even if a train got tripped, the motorman would simply reset the trip cock and continue on his way (no drug/alcahol tests at the end of the line). Also they had far fewer timers. Ironically, despite all this, there wasn't a single passenger fatality on the NYC subway system due to a "railroading" type accident (collision or derailment) between the late 1920's and 1970.
