Holding lights are supposed to be illuminated before or while a train arrives in the station, not after. Once indication is given to the T/O, the operation is finalized.

It's happened to me many times, particularly at Jay Street. The holding lights are right over the C/R's head, so if you're not looking up you'll miss them.

The towers have recently started withholding the lineup so crews cannot run holding lights in stations.
